Job Description:
A company is looking for a Care Coach II to support family
caregivers and their care recipients.
Key Responsibilities
Engage, motivate, coach, and educate caregivers via phone and
digital technology
Manage a caseload and leverage technology for effective coaching
and information referral
Assess caregiver and care recipient needs and assist in creating
care plan goals
Required Qualifications
4-6 years' experience in health coaching and/or health
care/wellness
Comprehensive knowledge of industry concepts and principles
Proven experience accessing and locating community resources
Solid understanding and experience in care planning/case
management
Multilingual skills are highly desired
