Job Description:
A company is looking for a Care Coordinator to provide care
coordination for Medicaid, Medicare Advantage, and Dual Eligible
Special Needs Plan members.
Key Responsibilities
Conduct outreach to educate members about program benefits and
facilitate care coordination
Assess member status and develop care plans to address individual
goals and needs
Manage a caseload, ensuring timely communication and coordination
with healthcare providers
Required Qualifications
2+ years of experience in care coordination, case management,
community health work, or related fields
Experience working with Medicaid and/or Dual Eligible patients
Remote work experience preferred
Experience documenting case notes in care management or electronic
health record platforms
Dedicated home office for remote work
