ResponsibilitiesVenice Family Clinic is a community health center whosemission is 'To provide quality primary health care to people in need.' Venice Family Clinic cares for over 28,000low-income patients in over 120,000 visits annually who might otherwise gowithout critically needed health care services. Venice Family Clinic has elevenclinical sites with over 350 staff, 1,700 volunteers, and a budget of $50 million. We provide high quality, comprehensiveprimary and specialty health care, dental, mental health, substance usetreatment and child development services to underserved individuals andfamilies on the Westside of Los Angeles.As part of our affiliation with the David Geffen School of Medicine atUCLA, Venice Family Clinic is involved in the training of residents and fellowsin primary care and many specialties.Venice Family Clinic also trains residents through partnerships withKaiser Permanente and Cedars-Sinai Medical Center.The Healthy Food Coordinator works within amulti-disciplinary health care team to deliver comprehensive services forpatients who need resources related to food and nutrition in a primary caresetting. Acting as the primary contactperson for the program, the Healthy Food Coordinator is responsible for the assessment,development, coordination, and implementation of food security programming andresources tailored to the needs of the patient and the patient's family servedat the Venice Family Clinic. The HealthyFood Coordinator is in charge of building the capacity of the food securityprogram, which includes working with internal and external agencies. They will provide guidance for fooddistribution events as well research and maintain resources of food programs inthe area. The Healthy Food Coordinatorwill provide patient education related to nutrition needed to learn and grow,increasing patients chances for success in all aspects of life. The Healthy Food Coordinator will manage theTeaching Kitchen and food carts, providing food demonstrations and volunteersneeded to conduct food demonstrations.The Healthy Food Coordinator will also manage the Food Pharmacy and theFree Food Market, overseeing volunteers to assist in the food distributionevents.Responsible for carrying out Food Security activities,including nutrition education, cooking classes, and Produce Markets. Mange the Teaching Kitchen food demonstrationsand volunteers needed to conduct food demonstrations. Manage the food cart(s) demonstrations at sitesand volunteers needed to conduct food demonstrations. Lead programming related to Produce Markets andfood distribution. Lead food and beverage demonstrations andtaste-tests to promote healthy eating, increase consumption of fruit andvegetables, and promote healthier beverage choices. Provide nutrition, physical activity, and skillsbuilding opportunities to patients participating in food securityprogramming. Manage interns/volunteers that are involved withfood security programming. Train and guide Health Educator(s) andinterns/volunteers involved with food demonstration. Prepare materials and equipment prior to theclasses/events. Oversee and manage food security program needssuch as ordering materials, purchasing snacks/food for classes,recruiting/coordinating volunteers, tracking visits/attendance, pre/postdata and securing space to conduct the sessions/classes. Develop appropriate targeting criteria andmechanisms to ensure the needs of identified groups are met. Assist with implementation of culturally relevantand linguistically appropriate materials for patients.Work closely and communicate effectively withclinical and non-clinical staff to inform them about food security servicesavailable and be informed of other clinic services available for thepatients. Provide inputin the program development and interventions.Build and enhance a mutual and interactiverelationship with residents of our service area communities, and variouscommunity-based organizations, including schools, day laborer centers, andother organizations. Provide information about the Clinic to communitymembers and serve as a liaison between the Clinic and the community. Identify new and maintain existing relationshipswith community organizations where outreach efforts may be conducted and foodsecurity program workshops can be offered for the community at large. Develop and distribute outreach flyers. Attendoutreach events as assigned by the Director of Health Education or asneeded. Document numbers of individuals reached throughoutreach activities. Conduct presentations at community-basedorganizations when needed.Monitor and evaluate targeted interventionpractices, contract requirements, and submit complete forms in a timelyfashion.Coordinate and manage food security servicestracking and submit monthly reports to the Director of Health Education.Develop and manage program-tracking systems forbudget management and productivity. Collaborate with other departments to keep up todate food resources. Conduct reminder and follow-up phone calls toparticipants of our food security program.Attend and actively participate in staff meetings. Refer patients to other clinic services andcommunity agencies, as needed.Maintenance of open, effective communication and interaction amongthe programs and partners; engagement of parents/families. Other duties as assigned withing Venice Family Clinic.QualificationsMaster's degree in Public Health, Nursing,Nutrition or related field OR a Bachelor's degree in Health or a related field and at least twoyears of experience in a public health or primary health careorganization - Required.Working knowledge of food security issues andprograms including nutrition, lifestyle and behavioral issues - Required.Bilingual English/Spanish skills - Required.Ability to establish and maintain cooperativeworking relationships with physicians, clinic staff, patients andvolunteers - Required.Ability to work effectively as a member of aninterdisciplinary team - Required.Knowledge of the Latino culture and diet - Required.Excellent communication and organizational skills - Required.Ability to effectively communicate with a widevariety of people of various educational and socio-economic backgrounds - Required.Ability tofunction efficiently and independently, and to plan and prioritize dailywork activities to meet clinic goals and objectives - Required.Ability to interact with staff to promoteinterdepartmental communication - Required.Demonstrated proficiency in using Microsoftapplications such as Word, Excel and Access - Required.Ability to travel between clinics and communityorganizations to conduct food security programs, presentation, services andoutreach - Required.UCLA is an Equal Opportunity/Affirmative Action employer.
